LOGOTHERAPEUTIC INTERVENTIONS, THE COURSE OF ILLNESS AND IMMUNOLOGY IN HIV PATIENTS

Abstract
As far as AIDS is concerned, there seem to be many factors contributing to its various stages of evolution. The influence of the psychic conditions on the immunological situation has already been widely studied. Our longitudinal research demonstrates how emotional stability, in turn, may be achieved when an inner meaning is realized, also thanks to a psychotherapeutic support. Contrariwise, a state of existential frustation has a negative influence on the immune system.
The new feature of this research consists in the periodic control of the ematochemical tests provided by the hospital referring to the sample groups, compared with psychological tests.

Zusammenfassung
Im Fall der Immunschwächekrankheit AIDS scheinen viele Faktoren an der Entwicklung der verschiedenen Stadien mitzuwirken. Der Einfluß des seelischen Befindens auf die Immunlage ist schon mehrfach untersucht worden. Unsere Verlaufsstudie zeigt, wie emotionale Stabilität erreicht werden kann, wenn - etwa auch aufgrund therapeutischer Unterstützung - die Situation in sinnvoller Weise gemeistert wird. Umgekehrt läßt sich der negative Einfluß existentieller Frustration auf die Immunlage nachweisen.
Die Besonderheit dieser Untersuchung liegt im Vergleich zwischen den regelmäßig von der Klinik erhobenen Blutbefunden und den Ergebnissen psychologischer Tests.

ZUORDNUNG DER KLINISCH-DIAGNOSTISCHEN LEITLINIEN DER ICD-10 ZUR LOGOTHERAPEUTISCHEN TERMINOLOGIE

Abstract
This is an attempt to correlate the principal diagnostic concepts of logotherapy with the categories introduced by the "International Classification of Psychic Disorders" (ICD-10) published by the World Health Organisation.
The authors of ICD-10 are not shy in admitting that disorder is a somewhat inexact concept. The expression is used to denote "a clinically discernible complex of symptoms and disordered behavior in combinat.ion with stress and with an impairment of certain functions. Social deviations or conflicts as such shall not be considered as psychic disorders."
The concept of "disease" was eliminated, and so was almost any kind of differentiation between psychic disorders on the basis of their
origin. ICD-10 is based on a purely descriptive-phenomenological analysis, as opposed to an etiological-pathogenetic one.
The reason for this is purely pragmatic: only in this manner the representatives of various psychotherapeutic schools were able to arrive at a compromise in defining a set of common diagnostic criteria. As an example, the classification group F0 is defined as comprising all disorders "that may be related to a distinctively organic cause". However, the doubtlessly endogenous factor in schizophrenia, in certain affective disorders, in cognitive impairment and in some developmental disorders is completely disregarded here.
Thus the specific sensitivity of logotherapy for that dimension of human existence which is affected in a given crisis is quite lost. The important discrimination between depressions that are of endogenous, exogenous/reactive, or noogenic origin - each requiring a completely different therapeutic approach - is missing.
In this context the following defects of the IDC-10 categorisation must be mentioned:
- The description and definition of noogenic neuroses and depressions is missing.
- The description and definition of somatogenic pseudoneuroses is missing.
- The description and definition of iatrogenic neuroses and traumata is missing.
- ICD-10 does not distinguish between psychogenic diseases with somatic effects (e.g. cardial neurosis) and psychosomatic diseases (e.g. gastric ulcer). Both are categorised under the new concept of "somatoform disorders and non-organic functional disorders". In this way the existence or non-existence of a previous organic defect prior to the appearance of psychic triggers is not registered.
- ICD-10 lists pathological reactions to serious stress; however, the equally important group of pathological reactions to sudden stress relief is disregarded. Also, the concept of "adaptational disorder" for "states of subjective suffering and emotional impairment following decisive changes in life" is questionable. Explicitly mentioned is the case of mourning, which leads to the consequence that the grief of a wife mourning for her husband would have to be categorised as an "adaptational disorder" with regard to her new status as widow.

LOGOTHERAPEUTIC AND SCHICKSALSANALYTIC APPROACHES TO DISABILITY AND TO CHANGE, AND THEIR RELEVANCE FOR SOCIAL WORK WITH THE DISABLED.

Abstract
Social workers and other helping professionals in their daily work and struggle on behalf of their clients often experience a sensation of
apathy, boredom, and depression from the constant efforts they expand in meeting other people's needs, while neglecting their own.
The phenomenon of "burnout", which expresses itself in frustration, lack of creativity, etc., is dangerous, as it stifles motivation to serve in the noblest sense of the word. The theories of Frankl and Szondi about "forced fate" and "selected fate", and about change and disability are particularly helpful to these workers to fill up their own spiritual wells and to face their tasks with renewed vigor and spirit. Cases illustrating these concepts at work are provided to augment the philosophical, therapeutic, and humanitarian approaches contained in the writings of Frankl and Szondi.

Sozialarbeiter und andere Angehörige der helfenden Berufe empfinden häufig in der alltäglichen Arbeit und Mühe im Dienst ihrer Klienten ein
Gefühl der Apathie, der Langeweile und der Depression. Dies ist eine Folge ihres andauernden Bemühens, die Bedürfnisse anderer Menschen
zu befriedigen, während sie meist ihre eigenen Bedürfnisse zurückstellen. Das Phänomen des "Burnout", das sich in Frustration, Mangel an
Kreativität usw. äußert, ist so gefährlich, weil es die Bereitschaft zum Dienst im edelsten Sinn dieses Wortes schwächt.
Die Theorien Frankls und Szondis über "aufgezwungenes Schicksal" und "selbstgewähltes Schicksal" sowie über Veränderung und Behinderung sind von besonders großem Wert für diese Helfer, weil sie ihnen dazu dienen können, ihre geistigen Ressourcen wieder aufzufüllen und ihre Aufgabe mit erneuter Kraft und Energie wahrzunehmen. Am Beispiel einiger Fälle, die diese Konzepte gut illustrieren, werden wir die philosophischen, therapeutischen und humanitären Ansätze in den Werken Frankls und Szondis untermauern und erweitern.

DIE BEDEUTUNG DER EXISTENZANALYSE UND LOGOTHERAPIE FÜR MENSCHEN MIT (SOGENANNTER) GEISTIGER BEHINDERUNG

Zusammenfassung
Es werden aus dem umfangreichen Werk von Viktor Frankl jene Aspekte herausgestellt, die für die Heilpädagogik der Achtung, für die therapeutische und helfende Begleitung und Erziehung der Menschen mit kognitiven Beeinträchtigungen, die man in der Regel als `geistig' behindert bezeichnet, bedeutsam sind. Frankls haltgebende und wertgeleitete Sinn-Lehre gibt der heilpädagogischen Praxis, die ich als vertiefte Normalpädagogik verstehe, bedeutsame Impulse. Die primäre Motivationskraft jedes Menschen - auch des kognitiv beeinträchtigten Menschen - ist die Suche nach dem Sinn.
Dafür sprechen die vorgestellten Beispiele. Wir verdanken sie den Lebens- und Berufserfahrungen von Dorothea Schmidt-Thimme, die sie vor allem mit ihrer `geistig' behinderten Tochter und mit Angehörigen der Werkstatt für Behinderte in Marburg sammeln und ordnen konnte. Frau Schmidt-Thimme ist als Mitbegründerin der deutschen "Lebenshilfe für geistig Behinderte e.V." (1958) ihrer persönlichen Lebenshilfe-Praxis treu geblieben.

Abstract
We will point out those aspects of Viktor Frankl's opus that are of specific importance for a "occupational therapy in dignity", i. e. for the therapy, supportive treatment and education of persons with cognitive impairment. Frankl's value-oriented theory of meaning provides important stimuli for this field of practice, which I perceive as an extension of normal pedagogy. The primary motivational force in any human being - and this holds also for persons with cognitive disadvantage - is the search for meaning.
This is corroborated by the cases presented here. They are taken from the host of professional and life experiences of Dorothea Schmidt-Thimme, who has a handicapped daughter, and who has founded and directed a workshop for the impaired in Marburg, Germany. Ever true to her commitment for assisting the physically and mentally impaired, she is also co-founder of the German "Life Skills Assistance Association" (1958).

IDENTITY CRISIS IN SCHIZOPHRENIA AND WAYS OF OVERCOMING IT

Abstract
This paper, based on the V.E.Frankl's logotherapy and the identity theory of D.P. McAdams, discusses the problem of identity crisis in schizophrenia and puts the question of the possibility of its reachievement after the active phase of this illness.
The investigation of 30 case studies (applying mainly biographical methods and projective techniques) resulted in the conclusion that religious beliefs of persons suffering from schizophrenia may help them to find meaning of their own illness and make it easier for them to accept the illness and to integrate it with the rest of their life story.
The study also showed that this help is not equally efficient in every case. That means, it is not enough to be a believer and to "use" the beliefs, if necessary, in order to understand the meaning of different events in life. The most important thing seems to be the way in which the reality, which is the object of one's beliefs, is experienced, as the way in which one understands their life depends on how this reality is seen and understood.

Zusammanfassung
Die Arbeit, der die Logotherapie V. E. Frankls und die Identitätsauffassung von D. P. McAdams zugrundeliegen, befaßt sich mit dem Problem der Indentitätskrise in der Schizophrenie sowie mit der Frage nach Möglichkeiten deren Wiederaufbaus nach Rückgang der aktiven Phase der Krankheit.
Die Ergebnisse der empirischen Untersuchungen (30 vorwiegend nach dem biographischen und projektiven Verfahren durchgeführte Studien) lassen feststellen, daß der Vorgang des Wiederaufbaus der Identität bei der Remission der Krankheit zwar schwierig, aber möglich sei und große Bedeutung dabei dem religiösen Glauben des Kranken zukommt. Dieser Glauben, indem er den Sinn des Leidens erkennen hilft, räumt eine potentielle Chance ein, die Krankheit zu akzeptieren und sie mit dem sonstigen Leben zu integrieren.

MEANING: AN OUTWARD SEARCH

Abstract
The author, a 21-year-old university student, was introduced to logotherapy in his first psychology course. Reading Man's Search for Meaning transformed his outlook and here he elucidates two key areas: (1) selfism and the search for meaning in American culture; and (2) a personal retrospective understanding of the death of his father and the way in which his mother prevailed in her suffering. Implications for the inward and outward search for meaning are explored.

Der Autor, ein 21-jähriger Universitätsstudent, begegnete der Logotherapie in seiner ersten Psychologie-Vorlesung. Die Lektüre von Man's Search for Meaning veränderte seine Weltsicht, und im folgenden beschreibt er zwei Aspekte dieser neuen Einsicht: (1) Selbstbezogenheit und die Suche nach Sinn in der amerikanischen Kultur; (2) eine persönliche, von neugewonnenem Verständnis geprägte, Rückschau auf den Tod seines Vaters und die Weise, in der seine Mutter sich in ihrem Leid bewährte. Die Bedeutung dieser Überlegungen für eine nach innen bzw. nach außen gerichtete Suche nach Sinn werden erörtert.

Using examples from the theory and practice of management, he demonstrates the distiction between `meaning', 'ends', and 'means'. With this in mind, he approaches such areas as the working place, education, historical science, and cultural science.
